Several important changes in the Passport Program will be effective February 1, 2008.

•  New Passport Application Forms -

When using forms DS-11, DS-82, DS-4085, DS-5504 and DS-3053 make sure you are using the most recent form dated 2/2008

•  New Passport Card –

Customers will now have the option to apply for a Passport Card. The Passport Card may ONLY be used for land and sea travel between the United States and Canada, Mexico, Bermuda, and the Caribbean. These cards are valid for 10 years for adults, 5 years for minors (Age 15 and under), are wallet size and cost $45 for adults and $35 for minors for first time applicants. Adults 16 and over that currently have a fully valid passport can apply for the Passport Card using a DS-82 form and pay $20.

•  New Fees

The total cost for a passport with Regular processing time is now $100. The U.S. Department of State receives $75. and the Passport Acceptance Agency receives $25. Renewal by Mail (Form DS-82) is $75.

•  Parental Consent Age Change

Two parent consent is required for minors under the age of 16. If one parent is unable to show in person at the Acceptance Agency form DS-3053 may be used by the absent parent.
